APPOINTMENTS

If you need to schedule an appointment, our office staff is ready and available to assist Monday through Friday between 8:15am – 5pm. Please realize, our staff will need to ask various questions to make certain we have the crucial information needed for you to take full advantage of your time with us. Therefore please have your personal data, physician's contact information and insurance card readily available.

For your convenience, we do our best to conduct insurance verification and pre-certifications prior to your appointment. However, keep in mind; if your insurance company requires pre-authorization from your primary care physician, it is in your best interest to obtain this information prior to your scheduled appointment so as not to delay your treatment. We recommend that you arrive 15 minutes early for your first appointment so that any last minute issues can be comfortably addressed.

In order to make your visit with our team an easy and pleasant one, we have provided most of our new patient forms online. We encourage you to print the corresponding forms, fill them out in their entirety and bring them with you to your visit.

INSURANCE

Please bear in mind, insurance plans are constantly changing with new policies, procedures, provisions and restrictions. Therefore, we recommend you call your insurance company for up-to-date plan coverage.

We will always work hard to adhere to the requirements of your insurance plan. In order to help you maximize your benefits, please bring your current insurance card to every visit. If you have any questions, please feel free to call our insurance and billing department at 941-957-1514. Our insurance specialist will be happy to assist you.

PAYMENTS

As a courtesy, we will file your insurance claim for you. Each patient is responsible for their deductible, co-pay and co-insurance at the time of service. In the event that you have a question regarding billing or payment information, we have office staff on hand to assist. Please ask for our insurance department and we will do our best to resolve any concerns.

WAITING TIME

In an effort to be efficient, we see patients by appointment. Although it is our goal for patients to be seen promptly, we sincerely appreciate your understanding and patience during emergency situations that may cause wait time. Remember, if you have an emergency- we will extend the same privilege to you.

QUESTIONS/CONCERNS

We want you to feel comfortable calling our office regarding your orthopædic needs. In order to assist you in a timely manner, we encourage you to call us during office hours while medical staff is available. Dr. Moor and his medical team see patients throughout the day but will answer all emergent questions as quickly as possible. We do our best to answer all questions within a 24 hour period.

AFTER-HOUR EMERGENCY CARE

If you are an established patient and truly have an orthopædic emergency after hours, please call 941-320-1224 immediately. Your call will be promptly returned.

PRESCRIPTIONS

Due to the guidelines governing prescription medications and the need to avoid hazardous drug interactions, all prescription refills must be handled during regular office hours when medical staff is available to pull your records.

We recommend that you (or your pharmacist) call our office to request a refill at least 24-48 hours before you finish your prescription. We want to make sure there is adequate time to provide you with your medicine. All refill requests are reviewed by a physician, and then either approved or denied. If your prescription refill cannot be approved, one of our office staff will promptly contact you.

REQUEST FOR MEDICAL RECORDS

In the event you need a copy of your medical records to be faxed or sent to another physician, please give us at least 48 hours notice. If you need x-rays or MRI films released to another party, we are more than happy to comply. All medical records are the property of Advanced Sportsmedicine Center and therefore, depending upon the request and party involved there may be a small fee incurred.

CONFIDENTIALITY

Rest assured that your medical records are completely confidential. This information will only be released upon your written request and approval.
