APPOINTMENTS
If you need to schedule an appointment, our office staff is ready and available to assist Monday through Friday between 8:00am-5pm. Please realize, our staff will need to ask you a various questions in order to make certain we have the crucial information needed to ensure your appointment goes smoothly.
We recommend that you arrive 15 minutes early for your appointment, so that you can comfortably fill out the required paperwork and allow us to make any necessary phone calls regarding insurance verification or pre-certification. (Please keep in mind, if your insurance company requires pre-authorization from your primary care physician, it is in your best interest to obtain this information prior to your scheduled appointment – so as not to delay your treatment.)
In order to make your visit with our team an easy and pleasant one, we have provided most of our new patient and established patient forms online. We encourage you to print the corresponding forms, fill them out in their entirety and bring them with you to your visit.

WAITING TIME
In an effort to be efficient, we see patients by appointment. Although it is our goal for patients to have no wait or a minimum wait at most, we sincerely appreciate your understanding and patience during emergency situations. Please know, if you have an emergency – we will extend the same privilege to you.
PRESCRIPTIONS
Due to the guidelines governing prescription medications and the need to avoid hazardous drug interactions, all prescription refills must be handled during regular office hours when medical staff is available to pull your records.
We recommend that you (or your pharmacist) call our office to request a refill at least 24 hours before you finish your prescription. We want to make sure there is adequate time to provide you with your medicine. All refill requests are reviewed by a physician, and then either approved or denied. If your prescription refill cannot be approved, one of our office staff will promptly contact you.
INSURANCE
We will always work hard to adhere to the requirements of your insurance plan. In order to help you maximize your benefits, please bring your current insurance card to every visit. If you have any questions, please feel free to call us at 941-957-1500. Our insurance specialist will be happy to assist you.
